How do I start a server?

I am not new to Tremulous but I do not know as much about servers as I would like.
I know there is a Create Server button but does that way put your server on the server list?
Does Dedicated LAN or Internet affect if it is on the server list. I have a router and am not sure if others will
be able to join my server. Do all newly created servers go to the top of the list. Thank you
also what is this thing I am reading in Server info about TJW, what is it? Please Help Thank You

If he's too lazy/dumb/whatever to look in the "Servers" forum and read the threads with bold titles telling you how to set up a server, what makes you think he would read the FAQ?
